摘  要：[目的] 评价关节镜下后侧入路治疗腘窝囊肿的临床疗效及其与传统开放手术的比较。[方法] 将40例腘窝囊肿患者分为两组：A组21例，关节镜下后侧入路治疗，按Rauschning和Lindgren腘窝囊肿分级法Ⅰ级2例，Ⅱ级9例，Ⅲ级10例；B组19例，传统开放手术治疗，Ⅰ级2例，Ⅱ级9例，Ⅲ级8例。记录并比较两组手术切口长度；囊肿复发率；恢复至0~Ⅰ级的好转率。[结果] 患者均获得8~18个月随访，平均12.7±2.4个月。A组手术切口长度1.3±0.2cm，显著低于B组6.1±0.5cm（P＜0.05)；A组术后复发率9.5%，B组10.5%，两组间无显著差异（P＞0.05)；A组术后好转率为90.5%，B组89.5%，两组间无显著差异（P＞0.05)。[结论] 关节镜下后侧入路治疗腘窝囊肿疗效令人满意，相比传统开放手术具有创伤小、恢复快的优势。马鞍山市人民医院骨科谢洋

Arthroscopic Treatment of Popliteal Cyst Through the Posterior Portal//XIE Yang，LI Su-wan，LIU Chao-hui，et al．Department of Orthopaedics，The People’s Hospital of Maanshan City，Maanshan，243000，China

Abstract：[Objective] To evaluate the effect of arthroscopic treatment of popliteal cyst through the posterior portal and to compare its effect with traditional open operation．[Methods] 40 patients were divided into two groups．21 patients in group A were treated with arthroscopy through the posterior portal．According to  Rauschning and Lindgrenand criteria of evaluation，2 patients was of grade Ⅰ，9 patients of grade Ⅱ，and 10 patients of grade Ⅲ．19 patients in group B were treated with traditional open operation．According to the same criteria of evaluation，2 patient was of gradeⅠ，9 patients of grade Ⅱ，and 8 patients of grade Ⅲ．Length of incision，recurrence rate，cure and improvement rate of the two groups were observed and compared respectively．[Results] All patients were followed up from 8 to 18months (12.7±2.4 months in average)．The average length of incision is 1.3±0.2cm in group A and 6.1±0.5cm in group B，the average length of incision in group A is significantly shorter than that in group B(P＜0.05)．The recurrence rate is 9.5% in group A and 10.5% in group B，there is no significant difference between two groups (P＞0.05)．The cure and improvement rate is 90.5% in group A and 89.5% in group B，there is no significant difference between two groups(P＞0.05)．[Conclusions] Good effect can be achieved in arthroscopic treatment of popliteal cyst through the posterior portal，which presents such advantages as less invasion and fast-recovery in comparison with the traditional open operation．
