[摘要] 目的：探讨应用改良器械行经皮颈椎间盘髓核摘除术（PCD）的安全性及效果。方法：自1994年8月开始，通过对人体颈部解剖与影像学断面图像的对照研究，自行改良设计制作了新的手术器械，以3条成年犬为实验对象，行经皮颈椎间盘穿刺操作，观察穿刺的安全性，于1995年5月应用于临床，我们采用钳取和环锯局部切割相结合方式实现突出部位直接减压并以双腔式冲洗吸引器行盘内冲洗和0.08 mKpa～ 0.1mKpa负压吸引技术。本文搜集了资料完整的252例PCD病例，共306枚椎间盘，均经5个月至2年以上随访，其中随访时间在4年以上的34例。结果：本组3条试验用犬36次经皮穿刺有1次回血。经皮穿刺椎间盘过程发现小许回血的13例，胸闷和心前区疼痛2例，撤针后重新穿刺均获成功。术后3个月效果评价：优186例，良51例，优良率达94.0%，未发现术后感染和其他严重并发症。结论  改良后的PCD施术方法和手术器械可提高手术的安全性和成功率，从而减轻了施术医师和患者的心理压力，有利于该疗法的推广。烟台毓璜顶医院介入治疗科姜文进

Percutanous cervical discectomy surgery with modified instrument and surgical method  

【Abstract】　Objective： To improve and enhance the success ratio of treatment based on research of the percutaneous cervical discectomy. Methods：In August 1994, we began research on human cervical anatomy and section image comparison and modified surgical instrument using an improved type of technique and three adult dogs. On the basis of this success, we began clinical application of this treatment in May 1995. Finger press and pliers press were used as local press method，associted with hollow needle puncture technology，Direct decompression with pliers and local ring-saw method to remove the protrusion part，which is sucted by double cavity wash-draw technique with a negative pressure of 0.08 mKpa～ 0.1mKpa. We have collected 252 integrated PCD cases and 306 intervertebral discs in this article. The follow-ups were from 5 months to 2 years or above. 34 of those cases had 4 years’follow-up. Result：There are slight blood reflux in  13 cases after the percutaneous puncture of intervertebral disc, while all of them succeeded after re-puncture. Appraisal of results 3 months after the procedure, 186 cases were excellent, with 51 cases were classified as good. Considered overall, the success rate amounted to 94.0%, There were no postoperative infection or other serious complications. Conclusion： The success rate of this procedure is enhanced by using this PCD method and operation instrument, with its subsequent reduction in psychological pressures on both doctor and patients. This therapeutics should be spread.
